Optima Care extends Integrated Commissioning, partnering with housing charity to offer truly personalised Supported Living

Optima Care is delighted to announce that it has partnered with registered charity Future Housing Solutions to provide personalised accommodation solutions for those with intellectual disabilities or a mental illness. The partnership continues to extend the Integrated Commissioning approach of our ‘Shine’ model of therapeutic care, supporting the pathway into community for individuals.

The partnership began in December with the delivery of managed tenancy arrangements for all of Optima Care’s existing Supported Living properties. The second phase, launching with immediate effect, will see Optima Care and Future Housing Solutions working together with individuals and care managers.

Future Housing Solutions has many years’ experience in sourcing the right environment that best meets the needs of its clients: taking into account location, type, cost and long-term sustainability.

Optima Care and Future Housing Solutions have developed a pathway to delivery that affords a fully tailored, person-centric service. Only once a client has had a full in-depth needs assessment carried out by both Optima Care and Future Housing Solutions will the sourcing of a suitable property commence.

Once suitable accommodation is sourced and developed according to individual requirements, Optima Care will design a bespoke care and support package.

This service is currently available across the South East.
